We all face many difficult transitions in our lives; you don't have to go it alone. Compassionate Transitions is focused on guiding individuals and families through challenging times.
We are a non-medical practice dedicated to providing education and support for individuals facing terminal illness, as well as their loved ones.
For those needing assistance with Advance Care Planning, we offer expert guidance and personalized information to help individuals make informed decisions about their future care.

Providing compassionate support for individuals and their loved ones during life's final journey. We ensure comfort, help find peace and acceptance, plan meaningful vigils, write obituaries and eulogies, create remembrances and legacy projects, and act as the eyes and ears for hospice - honoring each life with dignity, presence, and care.
Providing personalized guidance to help you make informed decisions and ensure your wishes are known. We create aging-well plans for peace of mind, including advance directives, identifying a medical power of attorney, and facilitating family meetings - empowering you to navigate the future with clarity and confidence.
Restoring respect, dignity, and compassions for our elders by understanding what brings value to each individual. We build trust and meaningful connections, develop personalized aging-in-place and living well plans, and provide advocacy and companionship at medical appointments - supporting a life of purpose and comfort at every stage.
After a career in healthcare, I found my true calling in building meaningful relationships to help people navigate some of life’s most challenging transitions.
I am a Certified End-of-Life Doula, Elder Care Doula, and Doula Care Consultant. I'm also a proud member of the International Doulagivers Membership Association and the National End-of-Life Doula Association. I currently serve as a hospice volunteer doula and was honored to be the 2024 recipient of the Donna West Memorial Award (North Region).
I am devoted to guiding individuals and families through life’s most tender transitions. With compassion and deep listening, I support those facing life-limiting illness in finding peace, closure, and dignity. I also help with Advance Care Planning to ensure your wishes are known, respected, and honored.
