Life is full of transitions, and some of them can feel overwhelming - but you don't have to face them alone. Compassionate Transitions is here to guide individuals and families through the aging process, advance care planning, and life's final chapter with compassion, dignity, and respect.
We are a non- medical practice dedicated to supporting the whole person - body, mind, and spirit, through three ares of care:
Whatever stage you or your loved one are in, we are here to help you navigate these important transitions with clarity, confidence, and compassion.
Compassionate Support at Life's End
Offering calm, knowledgeable guidance and emotional support for individuals and families as life's final chapter approaches, creating space for comfort, connection, and love.
Thoughtful Planning for Peace of Mind
Helping you reflect on what matters most and document your wishes through advance directives, medical decision-maker selection, and family conversations - so your loved ones can honor you with confidence and love.
Restoring respect, dignity, and compassion for our elders by understanding what brings value to each individual. We build trust and meaningful connections, create personalized aging-in-place and living well plans, and provide advocacy and companionship at medical appointments.
After a career in healthcare, I found my true calling in building meaningful relationships to help people navigate some of life’s most challenging transitions.
I am a Certified End-of-Life Doula, Elder Care Doula, and Doula Care Consultant. I'm also a proud member of the International Doulagivers Membership Association and the National End-of-Life Doula Association. I currently serve as a hospice volunteer doula and was honored to be the 2024 recipient of the Donna West Memorial Award (North Region).
I am devoted to guiding individuals and families through life’s most tender transitions. With compassion and deep listening, I support those facing life-limiting illness in finding peace, closure, and dignity. I also help with Advance Care Planning to ensure your wishes are known, respected, and honored.
It would be my privilege to walk alongside you and your loved ones during life’s most meaningful moments.
